- Nicknames, abbreviations, or initials are not acceptable unless used as
your legal name.
- REQUIRED:
Changes must be accompanied by evidence of current use of legal change.
Acceptable documentation includes: marriage license,
driver's license, court order, state identification card, social security
card or passport.
- If providing this documentation by mail, please send copy only.
- Return your request to the Office of the Registrar with appropriate required
documentation, or copy as directed.
